To support our growth and the increasing volume of client care in Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Recertification Nurse (RN) to join our agency and serve existing and returning clients throughout NorthWest Ohio.

  • The primary service area is focused in and around Tiffin, OH, however coverage up to a 2-hour radius of surrounding communities is required.
  • Qualified applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.

This is a long-term employment opportunity, offering a consistent schedule to fit your personal / family commitments, with no overnight shifts or overnight travel. Our preference is to staff this with one Full-time RN (preferred), but multiple Part-time RNs will be considered if necessary.

  • As a Full-Time position - pay options include 1) hourly and paid weekly (Fridays), or 2) a negotiated salary with minimum # recerts required per week.
  • As a Part-Time position - pay is hourly, every week.
  • Typical travel distances require a 30-60 minute daily radius (or less), with travel between 1-2 hours (max.) required on occasion. Overtime may be available but is not required.

More about the role

  • Exact travel locations and daily shifts are customizable based on your availability and willingness to travel.
  • Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).
  • Round-trip / Door-to-Door mileage reimbursement provided.
  • Services primarily include recertifications, however opportunities for starts of care (SOC), resumptions of care (ROC), and other assessments and services are available (not required), as well as new hire orientations and supervisory reviews.
  • At least 1 year of experience in home health is required, with 2-5 years highly desirable.
  • Must have experience with OASIS and NON-OASIS assessments.
  • 1 or more years of experience in wound care, lab draws, and OASIS assessments is a plus.

Compensation

  • Hourly Wage: $35 - $40/hour
  • Annual Salary (+ PTO): negotiable based on travel radius and agreed-upon average recertifications per week

Job Summary

As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willingness to travel to clients in multiple counties.
