Job Description

As an RN with Moonlight Home Health Care, you will:

• Conduct skilled nursing visits in clients’ homes
• Assess and monitor patients’ health status and document care
• Administer medications, IVs, wound care, and other clinical tasks as needed
• Collaborate with interdisciplinary team members (therapists, aides, physicians)
• Educate clients and families on care plans, disease management, and post-acute needs
• Report changes in condition and coordinate follow-up care
• Ensure compliance with agency policies, state regulations, and patient safety standards

Qualifications

• Registered Nurse (RN) license in the State of Ohio
• Minimum of 1 year of clinical nursing experience (home health experience preferred)
• Valid driver’s license and reliable transportation (for home visits)
• Strong critical thinking, communication, and documentation skills
• Compassion, patience, and a commitment to patient-centered care
