Job Description

As an RN with Moonlight Home Health Care, you will:

Conduct skilled nursing visits in clients’ homes
Assess and monitor patients’ health status and document care
Administer medications, IVs, wound care, and other clinical tasks as needed
Collaborate with interdisciplinary team members (therapists, aides, physicians)
Educate clients and families on care plans, disease management, and post-acute needs
Report changes in condition and coordinate follow-up care
Ensure compliance with agency policies, state regulations, and patient safety standards

Qualifications

Registered Nurse (RN) license in the State of Ohio
Minimum of 1 year of clinical nursing experience (home health experience preferred)
Valid driver’s license and reliable transportation (for home visits)
Strong critical thinking, communication, and documentation skills
Compassion, patience, and a commitment to patient-centered care
