What You’ll Do

• 🏡 Provide ongoing in-home assessments & nursing care
• 📝 Develop & manage individualized care plans
• 🤝 Educate and support patients & caregivers
• 👩‍⚕️ Collaborate with healthcare providers to ensure coordinated care
• 💻 Maintain accurate EMR documentation & ensure compliance
• ✅ Participate in quality improvement initiatives

What We’re Looking For

• 🎓 Current RN license in Ohio (required)
• 💉 1 year acute care or med-surg experience (home health preferred)
• 💻 Proficiency with EMR & Microsoft Office
• 🚗 Valid driver’s license & reliable transportation
