Long-Term Care Nurse (RN) | Elyria, Ohio

A rewarding contract opportunity is available for an experienced Registered Nurse specializing in Med Surg - Inpatient Rehab care. This role demands clinical expertise in rehabilitation and long-term care nursing, with responsibilities encompassing comprehensive patient assessment, care planning, and collaboration with interdisciplinary teams. The position operates within a 30-bed inpatient rehab unit and requires participation in providing advanced nursing services including IV starts, medication administration, central line management, wound care, and patient safety advocacy.

Key Responsibilities

• Conduct initial and ongoing patient assessments and develop individualized care plans.
• Administer medications safely and comply with state and organizational nursing protocols.
• Coordinate patient care across hospital departments and serve as a primary point of contact.
• Utilize medical equipment such as Hoyer lifts, Arjo devices, Alaris pumps, and wound vacuums.
• Advocate for patient safety and participate in quality improvement initiatives.
• Collaborate effectively with interdisciplinary healthcare teams.
• Employ evidence-based practices and integrate quality improvement into nursing care.
• Maintain professional accountability and delegate appropriately while overseeing outcomes.

Desired Qualifications

• Valid nursing license from the state of practice.
• Minimum of 2 years nursing experience, ideally in med-surg or rehab settings.
• BLS certification required.
• Proficiency with dysrhythmia assessments (study guide provided).
• Familiarity with IV therapy, central lines, wound care, and telemonitoring.
• Associate degree or diploma in nursing required; BSN preferred.
• Excellent clinical judgment and interpersonal skills.
