Provide skilled nursing care to patients in their homes as a Home Health Registered Nurse (RN), delivering personalized, high-quality healthcare that supports recovery and independence. This full-time, 13-week contract position involves managing a caseload of 2532 points per week, typically 35 patient visits daily, within a field-based setting in Cuyahoga Falls, OH. Use your clinical expertise to assess, plan, educate, and coordinate patient-centered care in collaboration with an interdisciplinary healthcare team.

Key Responsibilities

• Perform comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
• Administer skilled nursing interventions including wound care, medication management, IV therapy, and chronic disease oversight.
• Monitor patient progress, detect changes in condition, and communicate effectively with physicians and the care team.
• Educate patients and families on treatment plans, disease management, and preventative health strategies.
• Document care activities accurately and timely using Home Care Home Base (HCHB) electronic documentation.
• Collaborate with therapists, social workers, and home health aides to ensure seamless continuity of care.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Adhere strictly to regulatory requirements and agency protocols.

Qualifications

• Active RN license in good standing for practice within the state.
• Minimum one year of recent home health experience, with field experience preferred.
• Proficient in Home Care Home Base (HCHB) documentation.
• Experienced with OASIS assessment tool.
• Strong clinical assessment capabilities and critical thinking skills.
• Reliable transportation, valid drivers license, and insurance to support daily travel.
• Ability to work independently and manage a flexible, field-based schedule.
• Excellent communication, organizational, and time management skills.

Preferred

• BSN degree.
