Home Health Registered Nurse (RN) - Lakewood, Ohio

As a Home Health Registered Nurse, you will deliver skilled, compassionate care to patients in their homes, coordinating with interdisciplinary teams to promote the highest quality outcomes.

Schedule

• Monday to Friday, 8:00 AM 5:00 PM

Key Qualifications

• Active RN license in good standing in the state of practice
• At least one year of recent home health nursing experience, with field work preferred
• Proficient in Home Care Home Base (HCHB) and OASIS documentation
• Strong clinical assessment, decision-making, and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to function well independently, delivering patient-centered care on a flexible field schedule
• Excellent communication and time management skills

Preferred candidates will have a BSN, prior experience managing 2532 points per week, and familiarity with Medicare and home health compliance standards.

Core Responsibilities

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing (wound care, medication administration, IV therapy, chronic disease management)
• Monitor and evaluate progress, report changes to physicians and care team
• Educate patients and families on disease, medication, and preventative care
• Complete accurate documentation in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team (PT, OT, MSW, aides) to coordinate care
• Attend case conferences, staff meetings, and engage in ongoing learning
• Ensure compliance with regulatory and agency standards
• Travel daily within your assigned territorythe position offers mileage reimbursement
