Provide comprehensive home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members in Columbiana County, OH. Key Responsibilities

• Conduct annual home health risk assessments for Medicare Advantage members
• Evaluate prior and current medical history, medical conditions, and medication history
• Provide patient education on medical conditions and medications
• Identify gaps in care and recommend appropriate resolutions
• No treatment plan creation or medication prescribing required

Qualifications

• Valid Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months current NP experience preferred
• MSN degree from an accredited school of nursing
