Position Overview

We are seeking a compassionate and skilled Home Health Nurse (LPN/RN) to join our team and provide high-quality nursing care to patients in their homes. The ideal candidate will be dependable, detail-oriented, and dedicated to supporting patients’ health, safety, and independence. This role includes delivering direct patient care, following individualized care plans, collaborating with healthcare providers, and promoting patient well-being in the comfort of their home.

Key Responsibilities

Patient Care

Provide skilled nursing care according to physician orders and individualized care plans
Perform assessments of patient condition, including vitals, symptoms, and overall health status
Administer medications, injections, wound care, and other treatments within scope of practice
Monitor and document changes in patient condition and report concerns promptly

Care Coordination

Collaborate with physicians, therapists, and other healthcare team members
Participate in creating, updating, and reviewing patient care plans
Educate patients and families on disease management, medications, and self-care techniques

Documentation

Complete accurate, timely, and detailed clinical notes and reports
Maintain compliance with agency policies, state regulations, and medical standards
Ensure proper documentation of all treatments, observations, and patient progress

Safety & Compliance

Follow infection control protocols and maintain a safe care environment
Adhere to HIPAA and confidentiality policies
Ensure tasks are performed within LPN/RN scope of practice

Home Visits & Support

Travel to patient homes to provide scheduled nursing visits
Assist patients with maintaining independence and quality of life
Provide emotional support to patients and families

Qualifications

Required

Active state LPN or RN license in good standing
Current CPR/BLS certification
Valid driver’s license and reliable transportation
Strong clinical assessment and communication skills

Preferred

Prior home health experience
Experience with EMR documentation
Knowledge of home health regulations and standards

Physical Requirements

Ability to travel between patient homes
Ability to lift, push, or pull up to 30–50 lbs as needed
Ability to stand, bend, and provide hands-on care
