Position Overview
We are seeking a compassionate and skilled Home Health Nurse (LPN/RN) to join our team and provide high-quality nursing care to patients in their homes. The ideal candidate will be dependable, detail-oriented, and dedicated to supporting patients’ health, safety, and independence. This role includes delivering direct patient care, following individualized care plans, collaborating with healthcare providers, and promoting patient well-being in the comfort of their home.
Key Responsibilities
Patient Care
- Provide skilled nursing care according to physician orders and individualized care plans
- Perform assessments of patient condition, including vitals, symptoms, and overall health status
- Administer medications, injections, wound care, and other treatments within scope of practice
- Monitor and document changes in patient condition and report concerns promptly
Care Coordination
- Collaborate with physicians, therapists, and other healthcare team members
- Participate in creating, updating, and reviewing patient care plans
- Educate patients and families on disease management, medications, and self-care techniques
Documentation
- Complete accurate, timely, and detailed clinical notes and reports
- Maintain compliance with agency policies, state regulations, and medical standards
- Ensure proper documentation of all treatments, observations, and patient progress
Safety & Compliance
- Follow infection control protocols and maintain a safe care environment
- Adhere to HIPAA and confidentiality policies
- Ensure tasks are performed within LPN/RN scope of practice
Home Visits & Support
- Travel to patient homes to provide scheduled nursing visits
- Assist patients with maintaining independence and quality of life
- Provide emotional support to patients and families
Qualifications
Required
- Active state LPN or RN license in good standing
- Current CPR/BLS certification
- Valid driver’s license and reliable transportation
- Strong clinical assessment and communication skills
Preferred
- Prior home health experience
- Experience with EMR documentation
- Knowledge of home health regulations and standards
Physical Requirements
- Ability to travel between patient homes
- Ability to lift, push, or pull up to 30–50 lbs as needed
- Ability to stand, bend, and provide hands-on care
